Produce a report an owner can use to decide what to fix and how to verify it. Assess supplied findings against their evidence; do not assume that a scanner, another agent, or an impressive technique name has established a vulnerability.
Record assessment purpose, dates, target/revision, inspected paths or endpoints, test environment, authorized actions, tools and versions when known, and evidence origins. Preserve reported timestamps and time zones. A reproduced failure on a different version is supporting context, not verification of the target version.
Mark each item as an observed effect, source-supported inference, heuristic match, or unverified report. Ask for the smallest missing artifact needed to resolve an important ambiguity while continuing independent report work. Do not fetch confidential systems or run a test merely to fill an empty field.
Use stable IDs and the following fields. Markdown or JSON is appropriate; do not claim compatibility with an external schema unless you validate against it.

Do not manufacture a CVSS number. If the user requests CVSS, verify the applicable version, record the vector and assumptions, and calculate the score using a trusted implementation. Severity without a numeric score is adequate for ordinary triage.
These are triage definitions, not universal severity assignments. A catastrophic hypothetical with weak evidence must retain low confidence. A harmless test marker reaching a log is not automatically data exfiltration. A secret reference is not necessarily secret disclosure.
Trace the cause and control boundary for duplicates. Merge identical causes on the same target while retaining all evidence locations. Keep separate findings when fixes or affected trust boundaries differ. Do not merge independent vulnerabilities just because they share a category.
Check whether the evidence came from test code, an unused configuration, a quoted security example, a generated artifact, or an inactive deployment. Inspect whether the vulnerable path is reachable and whether another control prevents the stated effect. Review dynamic results for stale state, wrong identities, mocks that skipped the authorization layer, and resource-limit failures.
For an installation verdict, confirm the reviewed bytes match the candidate. For an incident conclusion, distinguish attempted, authorized, completed, and externally confirmed actions. Missing logs constrain the conclusion; they do not prove an event did or did not occur.
When raw evidence contradicts a worker's conclusion, correct the finding and explain the reason briefly. Retain unresolved disagreements as explicit questions rather than silently choosing the more alarming version.
Remove credentials, private keys, customer content, and unnecessary personal identifiers. Preserve field names, relative paths, destination origins, and synthetic placeholders needed to explain the mechanism. Store a protected evidence reference instead of pasting raw secrets into the report. Do not open or disclose an unrelated credential merely to confirm its presence.
Keep a report's intended audience and storage location within the user's requested scope. Drafting a report does not authorize emailing it, publishing it, filing a public issue, or forwarding telemetry. If a finding requires coordinated disclosure, prepare the evidence package for the authorized owner.
Lead with the highest-priority verified result and the practical next action. Include a short scope statement, findings, verification, and coverage. Use not tested, not applicable, review required, and no blocking issue found in reviewed scope accurately. Do not replace missing evidence with a numeric risk average.
For fixes that were authorized and performed, identify the actual patch and test outcome. A proposed mitigation is not a completed fix; a successful build is not a security regression test. State remaining verification before closing a finding.
